New Patient Information

Insurance We accept most major medical insurances.  When calling to schedule a new patient appointment, please be ready to give the scheduler your specific medical information as well as your Name, Date of Birth, Telephone Number and your Primary Care Physician (If any).  We ask that you bring your insurance card(s), picture ID, and bring your current medication list and/or the prescription bottles.

*If you are transferring care, please be sure to sign a medical release form from your former physician prior to coming to your first appointment to expedite the transfer process.
*HMSA Quest/HMO/Tricare Prime Patients: We require a referral from your primary care physician (Tricare Prime: A referral from Tricare is required) prior to the visit. Please have your PCP or Insurance Carrier fax the referral to (808) 526-0221 and ATTN: Referrals.  It is preferred that an appointment be made prior to sending the referral.

*Dr. Philip Kuo Sr accepts HMSA quest, Dr. Philip Kuo Jr accepts HMSA quest only in our Pearlridge location

*If you are a new patient for the allergy physcian(s), please stay off of antihistamines 5 days prior to your scheduled appointment.  Antihistamines include (but are not limited to) Allegra, Atarax, Benadryl, Claritin, Mucinex, Xyzal, Zyrtec and all generic brand equivalents. Nasal sprays are generally okay, as well as Prednisone and Sudafed.  If you have any doubts or concerns, please consult your physician or call our office to inquire.
