I lost my lab/test order slip, what do I do? No problem! Give us a call and we can write out a new order requisition for you from your physician when they are in the office.

May I get a copy of my test results? We would like for you to come in to discuss the results with the physician and we can give you a hard copy.  This also ensures that we are giving results to the correct person (confidentiality).

I ran out of my medication, help! We completely understand that life happens.  Give us a call and we can take the message and give it to your physician.  Don’t forget to call us 3 days prior to running out next time.  If you have not seen the doctor in over 6 months, you will need to come in to get reevaluated prior to getting a refill.  Medications may need to be reassessed and adjusted and our physicians can do that with you face to face.

I need a referral to a new physician, can I make the appointment? Your primary care physician (PCP) would like to evaluate you prior to being referred to another physician.  This is so that if any tests need to be ordered, we can do that prior to you being referred, and we can send the other doctor a letter from your physician beforehand.
